Pigmentation treatment

They are a result of sun exposure, which provokes creation of pigment. Skin pigmentation is a sign of premature aging due to damaged skin caused by the sun. Some pigment cells are destroyed, hence other create too much pigment to provide sun protection. Pigmentation is located in areas most exposed to the sun: face, decollete, shoulders and fists. Newest research says that wrinkles are not a sign of aging but pigmentation is, especially an uneven facial complexion with pigmentation. 

1. Age spots

Caused by sun exposure which provokes the creation of pigment. Skin pigmentation is on of the premature aging signs caused by sun damage. Some pigment cells are destroyed by the sun, while others create too much pigment in order to provide sun protection. Pigmentation is located in areas most exposed to the sun: face, decollete, shoulders and fists. Newest research says that wrinkles are not a sign of aging but pigmentation is, especially an uneven facial complexion with pigmentation.

Any body part can be treated. We are talking about pigmentation such as: lentigo, epheleides and actinic keratosis. Melasma doesn't belong in this group. Age warts in their initial stages, while in skin level do belong in this group. It is advised to protect the exposed body parts, throughout the year, with a high protective factor while clothes should be an additional protection as to prevent pigmentation development. Most pigmentation is just an aesthetic issue, while treatments have a medical role as well because by destroying pigmentation development of actinic keratosis and skin cancer is prevented. 

2. Other pigmentation

Can be a result of an inflammation or injury when the blood that entered the tissue doesn't dissolve completely. Consequently a bruise forms which does not retrieve completely giving your skin a brownish color. Certain drugs can also cause pigmentation. Sometimes by accident we can inject pigmentation, and we are not talking about tattoos. With bruises one should immediately start using a heparin cream to remove the blood and speed up its dissolution. With inflammations an anti-inflammatory therapy should be started as soon as possible so that pigmentation doesn't occur as a consequence.   

How to remove pigmentation?

Initially freezing with nitrous oxygen, peelings, abrasion and other were methods of choice. These methods are unpleasant and painful and oftentimes followed by complications. The greatest revolution was caused by devices for pigmentation removal with optical energy: lasers, IPL and AFT devices. All mentioned types of pigmentation are removed by same principle, i.e. treatments. 

There are two laser types, i.e. devices for pigmentation removal: selective and non-selective.

  • Selective devices – lasers, AFT and IPL devices which use optical energy to target just the pigment, which then darkens and crumbles within the following days. AFT 570 nm treatment and 532nm, 585nm, 755nm and 1064 nm lasers are used. The so called Q-Switch lasers used for tattoo removal are used for this purpose as well. There are other types of lasers that are not so commonly used. The IPL device has a 530 to 1000nm wave length span. These devices are used to treat changes which are in skin level.
  • Non-selective devices – erbium or Co2 lasers are used to mechanically destroy the tissue. The erbium laser is somewhat better because it causes lesser damage to the skin. They are used for changes above skin level. Fractional lasers which cause microscopic damages below skin level which are then ejected are a novelty. They are used for changes in skin level or just above skin level.
